I reviewed the Revised Work Plan for Decommissioning of Monitoring Well MW-15(Workplan),
received electronically in an email from ENSR, Chevron's consultant on 15 February 2007. The
Workplan was submitted in response to the initiation of a street widening project by general contractor
SD Deacon on the comer of Robinhood Drive and Pacific Avenue in Stockton, which encompasses
monitoring well MW-15. Based on the ENSR email, monitoring well MW-15 (Figure 2)will be
decommissioned on Wednesday, 21 February 2007 by overdrilling the well casing and grouting the
boring to subgrade of the construction project. The San Joaquin County Environmental Health
Department has received the drilling permit from ENSR, requested a check for the permit and
oversight costs, and approved the Workplan by email on 15 February 2007 (Attachment). In addition
to the SJCEHD requirements for the Workplan listed in the attached email, the Workplan is approved
with the following conditions:
• that the Investigative Derived Wastes (soil and groundwater) be characterized by laboratory
analyses for constituents of concern prior to proper disposal, and
• that the well decommissioning be coordinated with the SD Deacon.
The contact information for SD Deacon is Jeff Page, Superintendent, jeff.oaaea-deacon com,
office phone (916) 969-0900, fax (916) 729-0900, mobile phone (916) 869-4508. You may call me at
