Fresh asphalt had been forced into the void space surrounding the upper portion of the well casing in the well
box.
3.) The well cap was found sitting inside the well box.
4.)The top of the well casing did not appear to be damaged but the well cap was not on the top of the casing and
there was a significant amount of asphalt inside the well box and around the opening of the well casing. The
opinion of our field staff is that asphalt and or other debris may have fallen into the open well casing.
5.)An asphalt contractor( Cal Coating 209-667-8443 site former Jose Montero)was onsite repaving the parking
lot around the Macaroni Grill.
ENSRs actions upon notification:
1.) ENSR re-attached the locking well cap to MWA and placed a barricade over the well.
2.) 1 contacted Greg Hall this morning @ approximately 0930am to convey the situation to him.
3.) ENSR exchanged the well box lid for MWA with the well box lid for MW-3 (located in a planter)to temporarily
limit a trip hazard and possible additional damage to MW-1.
ENSs is very concerning that asphalt may have fallen in the well casing and degraded the groundwater in the
vicinity of MWA. Greg Hall said he will follow up with you to address our concerns regarding the asphalt and or
other debris that may have fallen into the open well casing. He will also address replacement or repair the well lid
for MW-1. Please contact Greg and myself no later than tomorrow(8/27/04) so that we can minimize any
potential impacts to groundwater as soon as possible.
Thanks
We all need lots of powerful long-range goals to help us past the short-term obstacles....Jim Rohm
Ben Heningburg
Sr.Project Manager
ENSR International, Inc.
10411 Old Placerville Rd., Ste 210
Sacramento, CA 95827
Ph 916.288.4315
Fx 916.362.8100
www.ensr.com
8/30/2004
