Mr. James Barton February 15,2007 Page 2 On February 6, 2007, ENSR field staff visited the Site to perform remediation system operation and maintenance(O&M). During that Site visit, ENSR observed the SD Deacon Corporation performing building tear-down activities on the former Union Bank building. Monitoring Well MWA 5 was not affected by the work observed on February 6,2007. ENSR field personnel marked the area surrounding well MW-15 with orange paint, notified the SO Deacon foreman not to damage the well, and provided ENSR's contact information. Scope of Work Proposed tasks include the following: • Pre-Field Activities • Well Decommissioning • Report Preparation TASK ONE-PRE-FIELD ACTIVITIES ENSR will update the site-specific Health and Safety Plan (HASP)which will address potential physical and chemical hazards associated with work at the Site and other health and safety considerations associated with well decommissioning. The HASP will be reviewed and approved by ENSR Health and Safety Management prior to commencement of field activities. All work by ENSR and subcontractors will subsequently be performed in accordance and compliance with the HASP. Following approval of this Work Plan, ENSR will mark and identify the well proposed for decommissioning (MW-15)and request an underground utility clearance and markout from Underground Service Alert(USA)at least 48 hours in advance of any subsurface activities, as required by law,to identify any man-made underground hazards. Concurrent with the submittal of this Work Plan ENSR submitted a well decommissioning permit to the San Joaquin County Environmental Health Department(SJCEHD). A copy of the application is located in Attachment C. After this Work Plan is approved and the permit is issued, ENSR will schedule Gregg Drilling, a State-of-California C-57 licensed drilling contractor, to perform the well decommissioning in accordance with the County and State Well Standards. TASK Two-FIELD ACTIVITIES Field activities will be performed under the supervision of a State-of-Califomia Professional Geologist. The SJCEHD and the Central Valley Regional Water Quality Control Board (CVRWQCB)will be notified at least two business days before field activities are begun. At the commencement of field activities, ENSR will: • Conduct a tailgate safety meeting at the Site; • Review the contents of the HASP with ENSR and subcontracted workers performing field activities; • Set up and demarcate the Exclusion Zone around the work area(s)to preclude access by anyone whose entry is unauthorized; ENSR (AECOM
