Mr. James Barton <br /> February 15,2007 <br /> Page 2 <br /> On February 6, 2007, ENSR field staff visited the Site to perform remediation system operation <br /> and maintenance(O&M). During that Site visit, ENSR observed the SD Deacon Corporation <br /> performing building tear-down activities on the former Union Bank building. Monitoring Well <br /> MWA 5 was not affected by the work observed on February 6,2007. ENSR field personnel <br /> marked the area surrounding well MW-15 with orange paint, notified the SO Deacon foreman not <br /> to damage the well, and provided ENSR's contact information. <br /> Scope of Work <br /> Proposed tasks include the following: <br /> • Pre-Field Activities <br /> • Well Decommissioning <br /> • Report Preparation <br /> TASK ONE-PRE-FIELD ACTIVITIES <br /> ENSR will update the site-specific Health and Safety Plan (HASP)which will address potential <br /> physical and chemical hazards associated with work at the Site and other health and safety <br /> considerations associated with well decommissioning. The HASP will be reviewed and approved <br /> by ENSR Health and Safety Management prior to commencement of field activities. All work by <br /> ENSR and subcontractors will subsequently be performed in accordance and compliance with the <br /> HASP. <br /> Following approval of this Work Plan, ENSR will mark and identify the well proposed for <br /> decommissioning (MW-15)and request an underground utility clearance and markout from <br /> Underground Service Alert(USA)at least 48 hours in advance of any subsurface activities, as <br /> required by law,to identify any man-made underground hazards. <br /> Concurrent with the submittal of this Work Plan ENSR submitted a well decommissioning permit <br /> to the San Joaquin County Environmental Health Department(SJCEHD). A copy of the <br /> application is located in Attachment C. After this Work Plan is approved and the permit is <br /> issued, ENSR will schedule Gregg Drilling, a State-of-California C-57 licensed drilling contractor, <br /> to perform the well decommissioning in accordance with the County and State Well Standards. <br /> TASK Two-FIELD ACTIVITIES <br /> Field activities will be performed under the supervision of a State-of-Califomia Professional <br /> Geologist. The SJCEHD and the Central Valley Regional Water Quality Control Board <br /> (CVRWQCB)will be notified at least two business days before field activities are begun. At the <br /> commencement of field activities, ENSR will: <br /> • Conduct a tailgate safety meeting at the Site; <br /> • Review the contents of the HASP with ENSR and subcontracted workers performing field <br /> activities; <br /> • Set up and demarcate the Exclusion Zone around the work area(s)to preclude access by <br /> anyone whose entry is unauthorized; <br /> ENSR (AECOM <br />
