Mr.James Barton <br /> February 15,2007 <br /> Page 4 <br /> TASK THREE—REPORT PREPARATION <br /> ENSR will prepare and submit a report to Chevron EMC,the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB), and the SJCEHD documenting the decommissioning <br /> activities. The report will include copies of the applicable permits,field observations, and tracking <br /> documents pertaining to transport and disposal of wastes generated in the well abandonment <br /> task. The report will be prepared under the supervision of and signed by a State-of-California <br /> professional geologist and/or professional civil engineer. A State-of-California Department of <br /> Water Resources form DWR-188 will be completed for the decommissioned well and filed <br /> separately with appropriate agencies. Copies of the completed Department of Water Resources <br /> (DWR)forms will be appended to the report. ENSR will submit all required electronic files <br /> necessary to comply with State-of-California GeoTracker requirements. <br /> Schedule <br /> ENSR proposes to commence work immediately upon receipt of regulatory approval. Well <br /> decommissioning will be completed before the start of street widening construction as approved <br /> by the City of Stockton. The proposed work including the report is estimated to require two to <br /> three days to complete following receipt of all required permits and dependant on subcontractor <br /> availability. <br /> Remarks I Signatures <br /> The proposals and interpretations in this work plan represent our professional opinions and are <br /> based, in part, on information supplied by the client. These opinions are based on currently <br /> available information and are arrived at in accordance with currently accepted hydrogeologic and <br /> engineering practices at this time and location.
