The <br /> Wards and Unocal sites are collectively referred to as the Site. The site location is shown on Figure 1. <br /> ' The approximate former locations of the destroyed wells are depicted on Figure 2. <br /> SCOPE OF SERVICES <br /> ' Pre-field Activities <br /> ' This section describes the activities performed to prepare for and complete the destruction of site <br /> wells. The pre-field activities we performed included the following: <br /> • Reviewed and utilized the site-specific health and safety plan prepared by AECOM on April 13, 2011, <br /> that addressed the use of personal protective equipment and worker safety during the proposed field <br /> activities. <br /> ' • Prepared a detailed description of well destruction methods for each type of well and obtained <br /> approval from the San Joaquin County Environmental Health Department(SJCEHD). <br /> • Obtained well destruction permit No. 0522692 from the SJCEHD and paid the requisite fees. <br /> ' A copy of the permit is in Appendix B. <br /> • Obtained traffic encroachment permit No. 75207 from the City of Stockton (the City) Public <br /> ' Works Department and paid the requisite fees.A copy of the permit is in Appendix B. <br /> • Provided the required 48-hour notification to the Underground Service Alert prior to performing <br /> the well destruction activities. <br />' 3160 Gold Valley Drive,Suite 800 0 Rancho Cordova,CA 95742-7515 0 Telephone 916.852.91 18 0 Fax 916.852.9132 <br />
