Nuel Henderson [EH] <br /> From: Nuel Henderson [EH] <br /> Sent: Friday, November 04, 2016 2:50 PM <br /> To: 'Scott Seyfried' <br /> Subject: Destruction of Vapor Extraction Wells EX-2 through EX-4 <br /> Scott, <br /> In accordance with our discussion today on the destruction of the vapor extraction wells referenced above under permit <br /> SR0076063, the approved destruction method is changed to: <br /> 1) Utilizing an 8-inch cutter to remove the concrete flooring around the well heads; <br /> 2) Removing the concrete, well box and bentonite seal occupying the upper 21 inches of the well traces; <br /> 3) Pulling the 10-foot length,2-inch diameter PVC casing and screen; <br /> 4) Removing the residual filter pack sand by hand-auger; <br /> 5) Grouting the remaining borehole with neat portland cement; <br /> 6) Finishing the top of the borehole appropriate to the floor of the building. <br /> If the casing cannot be fully removed: <br /> 1) The materials surrounding the casing shall be removed to a depth of 3 feet bsg at the eight-inch diameter at a <br /> minimum; <br /> 2) The well pressure grouted as best as possible; <br /> 3) The casing cut off 6 inches above the base of the excavation; <br /> 4) A portland cement mushroom cap formed to 6 inches above the top of the casing. <br /> 5) Finishing the upper 2.5 feet of the boring as appropriate to the floor of the building. <br /> Please call or email me with any comments or questions. <br /> Nuel Henderson <br /> Engineering Geologist <br /> (209)468-3436 <br /> 1 <br />
