(HFA) is pleased to present this well abandonment report to document the <br /> abandonment of ten monitoring wells at the above-referenced site (see Figure 1 - Site Location <br /> Map, and Figure 2 - Plot Plan). Ownership of the three remaining monitoring wells associated with <br /> the site (MW-2, MW-3, and MW-5) was transferred to the Lincoln Center Environmental Trust for use <br /> in its open environmental case. <br /> BACKGROUND <br /> The wells were abandoned in accordance with the letter dated June 1, 2009, and following the <br /> San Joaquin County Environmental Health Department (SJCEHD) concurrence with the methods <br /> of well abandonment outlined in a work plan for well destruction dated May 29, 2009, a revised <br /> work plan for well destruction dated June 18, 2009, a work plan addendum for well destruction <br /> dated August 12, 2009, and a work plan addendum for well destruction dated August 17, 2009 <br /> (see Attachment 1 for the agency correspondence) (ARCADIS, 2009a; 2009b; HFA, 2009a; 2009b). <br /> An encroachment permit was secured with the City of Stockton for the removal of wells MW-12, <br /> MW-13, and MW-14, which were installed in the sidewalk across Pacific Avenue from the Site (see <br /> Attachment 2 for the encroachment permit). <br /> WELL ABANDONMENT PROCEDURES <br /> On August 19 through 28, 2009, ten monitoring wells (MW-4 and MW-6 through MW-14) were <br /> abandoned in accordance with SJCEHD well destruction permit regulations and under the <br /> supervision of Mr. Mike Infurna of the SJCEHD (see Figure 2 for the abandoned well locations, and <br /> ENVIRONMENTAL SCIENTISTS GEOLOGISTS ENGINEERS <br /> Contaminated Site Assessment • Fixed & Mobile Remediation • Project & Program Management <br /> Ventura, CA Pleasanton, CA Redlands, CA Tempe, AZ Flagstaff, AZ <br /> 805-641-1056 800£72-0219 909-793.4571 480.505,3332 928-779-5447 <br /> j1 http://www.hfa.com <br />
