While trying to update our <br /> database, I discovered a slight problem that must be addressed before you can do <br /> any field work. <br /> Since this site was referred to the CVRWQCB back in 2005, the EHD has not <br /> conducted any chargable time to this site. As a result, the site is currently NOT on <br /> our accounting database. An EHD requirement to be taken care of PRIOR to any <br /> chargable time against the site. Chargable EHD time includes permit issuance <br /> (office time, service code on billing statements as "315"), consultant time ( service <br /> code 312) and field inspections (service code 310). <br /> With pending site work (May 23, 2008 work plan for the one push point at WOT-1 <br /> approved by CVRWQCB's James Barton in correspondense dated 28 May 2008), <br /> it's important that a billing party and an account code be set up now. <br /> Attached you will find a "Master File Record, (MFR)" in PDF format. Just print it off <br /> and fill it in please. This must be completed and signed by the billing party set up to <br /> pay for EHD time spent at/on this site and returned to the EHD with a prepayment <br /> for 3-hours time. At $105/hour, the prepayment amount due is $315. <br /> Once the account is set up for this site, I will be able to process boring permit <br /> applications and get you going on your way to completing the approved work plan <br /> and eventually, well destructions as a requirement for NFA. The EHD will send <br /> any charges accrued for this site for time spent over the 3-hour amount to the billing <br /> party identified on the MFR in monthly statements. <br /> Permit fees are separate from the MFR fee. Installation permit fee applications are <br /> $89 per parcel, per contractor, per work plan, within one year. Well destructions <br /> permits are $60 per parcel, per contractor, per work plan within one year. These <br /> fees should not be included in the check amount sent with the MFR, but sent with <br /> the permit application and LCD form separately as field work is approved by your <br /> lead agency for this site. <br /> 8/25/2008 <br />
