The following is an itemized list of hazardous materials violations that have not been <br /> addressed for HOGAN MFG INC as of March 30, 2020. <br /> Open violations from June 19, 2019 inspection <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map was missing emergency response <br /> equipments ( eye wash, first aid kit, spill kits, and fireextingushers), emergency shut offs ( diesel, argon, nitrogen, <br /> oxygen, electrical, water, and natural gas), and hazardous material storage areas. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer <br /> drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and <br /> storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list <br /> it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. the local Unidiefed Program Agency(UPA) phone <br /> number was not provided.
