The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ARP MINI MART CORP as of March 30, 2020. <br /> Open violations from November 02, 2018 inspection <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures are missing the local Unified Program Agency(UPA) phone number. Please <br /> add 209-468-3420 to line C3. In section G please make sure that the location of items checked are listed in the <br /> location section. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The facility information section was last on 1/25/2018 making it late. The inventory and <br /> plans section were last submitted on 5/16/2017. Please make a new submittal with all three sections for 2019 as <br /> soon as possible. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year.
