The following is an itemized list of hazardous materials violations that have not been addressed for PRO-TOUCH AUTO REPAIR as of March30, 2020. Open violations from October 09, 2018 inspection ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. Employee training records were unavailable for review at time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of the training records to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures, the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
