The following is an itemized list of hazardous materials violations that have not been <br /> addressed for SPEEDEE OIL CHANGE & TUNE-UP-MTCA as of March 31, 2020. <br /> Open violations from September 25, 2018 inspection <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. The site map is missing storm drain locations, loading <br /> and unloading area for the chemicals,A site map shall contain a north orientation, evacuation staging area, adjacent <br /> streets, internal roads, utilities shut offs, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures are not complete. Please fill out section 1,Al, C3, G, H1, and section J. <br /> Please list 209-468-3420 on line C3. Please fill out the locations in section G. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-Failed to annually review and certify that the business plan is complete,accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The business plan was not submitted for 2018 and was last submitted on 5/1/2017. <br /> Please submit the business plan for 2018. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
