SUBSTANTIVE REQUIREMENTS -5- May 21, 1992 <br /> FOR DDRW, SHARPE <br /> (d) The discharge of treated ground water from the DDRW, SHARPE - GWTS shall <br /> not exceed the design capacity determined during the POP of the System without prior <br /> approval from the Board,Department, and the US EPA. <br /> D. Provisions: <br /> 1. The Discharger may be required to submit technical reports as directed by the Board, <br /> Department, and the US EPA. <br /> 2. The Discharger shall comply with the attached Monitoring and Reporting Program, <br /> which is part of these substantive waste discharge requirements, and any revisions thereto <br /> as ordered by the Board, Department, and the US EPA. <br /> 3. The Discharger shall submit to the Board, the Department and the US EPA the <br /> following reports/documents for approval: <br /> (a). Within six months of signing the Record Of Decision (ROD) for the SHARPE's <br /> GWTS, a 65% design and layout of the entire SHARPE pump, treat, and injection <br /> system, which includes the following minimum items: <br /> 1. Numbers and locations (horizontally and vertically) of extraction, injection, and <br /> monitoring/observation wells. <br /> 2. Anticipated extraction and injection rates. <br /> 3. Proposed operation plan. <br /> 4. Proposed contingency plan for all 3 components: extraction, treatment, and <br /> injection for power outage and/or mechanical failure. <br /> 5. Geologic well logs and well development data sheets for all extraction, <br /> injection, and monitoring wells proposed for the SHARPE's ground water <br /> extraction and injection systems. <br /> 4. The Discharger shall submit to the Board, the Department, and the US EPA the <br /> following reports/documents for approval at least two weeks prior to the commencement <br /> of the Treatment Performance Evaluation phase of the SHARPE's ground water treatment <br /> system. <br /> (a) The Installation Data Report containing a record of the SHARPE's ground water <br /> treatment system as installed, including completed installation check lists for each <br /> component and the results of the hydrostatic tests conducted on the system's <br /> pipelines. <br /> (b) The Quality Assurance Project Plan containing quality assurance and quality control <br /> (QA/QC) procedures involved in the collection, preservation, packaging, and <br /> transport of samples, field testing, laboratory analyses,record keeping, data <br /> management, and chain-of-custody. <br />
