Initial <br /> and date the follow up actions. <br /> ITEM YESNO <br /> Aboveground Tanks and Piping <br /> Are any flanges or valves damaged or leaking? <br /> Is product present around the perimeter of any tank? <br /> Do any of the tanks shown sign of leakage? <br /> Is there any corrosion or abrasion of piping, particularly pipe supports? <br /> Do the tanks or their foundations or supports show signs of deterioration <br /> or corrosion? <br /> Fuel Dispensers <br /> Is there any evidence of abrasion or corrosion of the hoses? <br /> Is there any evidence of damage to or leaking from the dispensers? <br /> Containment Structures/Absorbent Materials <br /> Do the concrete containment structures show signs of deterioration, or <br /> oil sheen or discoloration? <br /> Are isolation valves functioning and closed? <br /> Is there an oil sheen or discoloration present on the drainage channel? <br /> Are the spill clean-up materials readily available and fully stocked? <br /> Provide any necessary comments or description of any corrective actions taken: <br /> 8411599 <br />
