The following is an itemized list of hazardous materials violations that have not been addressed for CALIFORNIA WELDING SUPPLY as of March30, 2020. Open violations from March 07, 2019 inspection Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. The Hazardous Materials Inventory forms for all chemicals were not complete. All of the chemicals have the old hazard categories listed. Please discard the obsolete federal hazard categories and the new federal hazard categories using an updated safety data sheet. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #6 - Failed to complete and/or submit a site map with all required content. The site map was not complete as part of the business plan. The site map is missing key or legend that defines FE, LA, and PL. The one storm drain and the utilities shut offs need to be identified on the site map. Please add the missing elements to the site map. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS) annually. The business plan was last submitted on 12/6/2017. Please make a submittal for 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
