The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Tire and Brake Pros as of April 01, 2020. <br /> Open violations from September 10, 2018 inspection <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms were not submitted for 2018. Please submit the chemical inventory for <br /> 2018. This information must be current and submitted as part of the business plan. Immediately log into the <br /> California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not complete. The local Unified Program Agency(UPA)phone number <br /> was not provided. Please list 209-468-3420 as your local UPA phone number on line C3. Only the first page of the <br /> emergency response and contingency plan was submitted. Please fill out and upload all 4 pages of the emergency <br /> response and contingency plan. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#9-Failed to annually review and certify that the business plan is complete,accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually.The last submittal date was 9/28/2015. Please make a submittal for 2018. <br /> The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year.
