The following is an itemized list of hazardous materials violations that have not been addressed for Tire and Brake Pros as of April 01, 2020. Open violations from September 10, 2018 inspection Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. The Hazardous Materials Inventory forms were not submitted for 2018. Please submit the chemical inventory for 2018. This information must be current and submitted as part of the business plan. Only the first page of the emergency response and contingency plan was submitted. Please fill out and upload all 4 pages of the emergency response and contingency plan. Please make a submittal for 2018. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year.
