SAN J O A � U I N Environmental Health Department <br /> —COUN i <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ON THE SPOT AUTO 710 E YOSEMITE AVE, MANTECA March 02, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item # Remarks <br /> 2 HSC 25505, 25508(a)(1) Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. This <br /> violation has been submitted on site. <br /> This is a Class II violation. <br /> 3 HSC 25507 Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a <br /> hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br /> materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br /> pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br /> The Business Owner/Operator Identification form was not submitted . This information must be current and <br /> submitted as part of the business plan. This violation has been corrected on site. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 110 gallons of used oil and 110 gallons of used anti freeze have not been reported on inventory. Any material that <br /> meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> This violation has been corrected on site. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> A site map was not submitted as part of the business plan. A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br /> a site map element is not applicable for your facility then list it on the map and label as"NA".
