The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ON THE SPOT AUTO as of April 08, 2020. <br /> Open violations from March 02, 2018 inspection <br /> Violation#10-Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, it was found that the business had a change in ownership and the updated information has <br /> not been submitted. Within 30 days of one the following events, business shall electronically up date the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> There were no employee training records for chemical awareness or handling. The business plan shall include <br /> provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#12-Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures,the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
