Provide the facility administrator Mrs. Bev Cortner and this office with <br /> at least 48 hours notice prior to commencing work. <br /> 2. During the actual work period one lane must be provided for traffic entering <br /> and exiting the front parking lot. Both entrance/exit points must be <br /> kept clear. <br /> 3. All work must be completed in one day and the premises are to be left <br /> clean and free of debris. <br /> 4. There should be no interference to pedestrians or traffic after work is <br /> complete, i .e. , the surface must be level with the surrounding pavement <br /> so as to not cause tripping hazard or obstacle to wheelchairs or guerneys. <br /> 5. The company performing the work must provide, in advance, proof of workman's <br /> compensation insurance, liability insurance and a hold harmless clause <br /> for Beverly Enterprises. <br /> If all of the above conditions can be met you have our approval .
