Hawes <br /> Director of Property Development <br /> Beverly Enterprises <br /> 11037 Trade Center Drive <br /> Rancho Cordova, California 95670-6193 <br /> RE: Installation of groundwater monitoring well at the Chateau Convalescent Hospital, <br /> 1221 Rose Marie Lane, Stockton, California <br /> Dear Mr. Hawes: <br /> On behalf of Exxon Company, U.S.A. and at the request of the San Joaquin County <br /> Public Health Services, Environmental Health Division, EA Engineering, Science, and <br /> Technology requests site access to install a groundwater monitoring well at the above- <br /> referenced site. <br /> As you know from our previous attempt to obtain access, this well will be installed as <br /> part of a continuing investigation of the former Exxon Retail Site located at 4444 North <br /> Pershing Avenue. <br /> Ms. Laurie A. Cotulla from the San Joaquin County Public Health Services, <br /> Environmental Health Division, also contacted you by letter regarding this matter (a copy <br /> of the letter is attached). <br /> Attached is Exxon's standard site access permission form, which, when signed by both <br /> parties, will permit us to install and sample the well, and hold harmless Beverly <br /> Enterprises and its representatives from any liability for damages arising in connection <br /> with the installation, maintenance, and removal of the well. <br /> Also attached are figures showing the tentative location of the proposed monitoring well <br /> and well construction details. <br />
