REVOCABLE PERMIT Ground Water Monitoring Well Permit No. : 88-01 Date: February 3 , 1988 Permittee: EA Engineering, Science & Technology, Inc. Western Reginal Operations 41 Lafayette Circle Lafayette, California 94549 In accordance with Ordinance 085-87 , passed by the Council of the City of Stockton on October 26, 1987 , a permit revocable at the pleasure of the Director of Public Works of the City of Stockton is hereby granted to: EA Engineering, Science, & Technology, Inc. to install and maintain four ground water monitoring wells. This permit is granted under the following terms and conditions: 1 . This revocable permit is granted subject to the Standard Conditions Applicable to Revocable Permits adopted by this City Council by Resolution No. 22, 578 on December 5 , 1960 , and on file in the Office of the City Clerk of the City of Stockton. 2 . The general location of each well (s) shall be as shown on the map attached. The precise location shall be determined in the field by the City of Stockton. 3 . The Permittee shall keep in full force and effect a Faithful Performance Bond in the amount of $2, 000 during the life of this permit. 4. In accordance with Section 1 (h) of the "Standard Conditions Applicable to Revocable Permits, " the permittee shall provide and keep in force during the term of the permit a policy of public liability insurance against liability for injuries to persons or property arising out of accidents attributable to the structure on City property, in amounts to be determined by the Director of Public Works, with limits of at least ONE HUNDRED THOUSAND AND NO/100 ($100, 000 . 00) DOLLARS for injury or death to any one person, THREE HUNDRED THOUSAND AND N0/100 ($300,000 . 00) DOLLARS for injury or death arising from any one accident or occurrence and TEN THOUSAND AND N0/100 ($10, 000. 00) DOLLARS for property damage.
