i The following is an itemized list of hazardous materials violations that have not been addressed for WESTERN TRUCK CENTER as of May25, 2017. Open violations from April 05, 2017 inspection Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Reportable amount of waste flammable liquid (gasoline)was found on site that has not been reported. Any r aterial that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. Violation #6 -Failed to complete and/or submit a site map with all required content. One of the site maps did not include a legend/key to identify the code used to denote hazardous material sto age containers and none of the site maps show the hazardous material/waste storage in the wash bay. A site m p shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access a d exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. Immediately log into the California Environmental Reporting System (CER ) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. Employee training records were not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Imme fiately provide employee training for appropriate personnel and submit a copy of the training records to the EHD. A 5ite map was not completed or submitted as part of the business plan. Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emrgency response plans and procedures, the inventory of information required by this article, and the site map requir d by this section shall be readily available to personnel of the business or the unified program facility with respons bilities for emergency response or training pursuant to this section.
