The following is an itemized list of hazardous materials violations that have not been <br /> addressed for INTSEL STEEL WEST as of May26, 2017. <br /> Open violations from February 03, 2017 inspection <br /> Violation#2 -Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. � <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and su it to <br /> the EHD for approval. <br /> i <br /> i <br /> Violation #3 - Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Bu ness <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a <br /> hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br /> materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperat re and <br /> pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.0a.gov/, <br /> enter the required information, and submit for review by the EHD. <br /> Violation #5 -Failed to complete and/or submit hazardous materials inventory for all reportable mat Mals on <br /> site. <br /> Reportable amounts of compressed oxygen, acetylene, diesel and mist lube were found on site that have n been <br /> reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's busin ss plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, a d the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emerg ncy <br /> response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. i <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or thrLPatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.caiepa.Ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. I <br /> Violation#8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program has not been submitted. Training programs for all employees and annual training, incl ding <br /> refresher courses, shall include safety procedures for the event of a release or threatened release of a ha ardous <br /> material, including, but not limited to, familiarity with the facility's emergency response plans and procedu s. These <br /> training programs may take into consideration of each employee. Immediately prepare or update the faci ty's <br /> training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca. ov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
