The following is an itemized list of hazardous materials violations that have not been <br /> addressed for TRACY76 as of April 07, 2020. <br /> Open violations from August 21, 2018 inspection <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for all chemicals are not complete. Please fill out the the new federal <br /> hazard category for each chemical. Please use the the chemical library in the California Environmental Reporting <br /> System or an updated Safety Data Sheet(SDS)to determine the appropriate hazard classes.This information must <br /> be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting <br /> System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The site map was not complete as part of the business plan. Please show the locations of all of the emergency <br /> response equipment(spill kits,fire extinguishers,absorbent). Please add a loading area for the fuel truck,fill point <br /> locations for the fuel tanks,and utility shut offs to the site map.A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets,storm and sewer drains,access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br /> a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into <br /> the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information,and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures are not complete. Section Al and section J are not filled out in the emergency <br /> response and contingency plan. Please check yes or no in section J and put the date in section Al business plan <br /> shall include the following emergency response procedures for a release or threatened release of hazardous <br /> materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
