representative of the designated facility which <br /> receives the used oil shall sign and date the i <br /> manifest, return two copies to the hauler, retain <br /> one copy, and send the original to the Department <br /> within 30 days. <br /> (11) All other manifesting requirements of conditions <br /> III.G.1, III.G.2, and III.G.3 shall be complied <br /> with unless specifically exempted under this ; <br /> subdivision. <br /> 5. Every registered hazardous waste hauler who transports used <br /> oil shall report to the Department the following ; <br /> information as part of the hauler's annual application for <br /> renewal of registration: <br /> (a) The shipping descriptions of used oil transported <br /> during the preceding year. <br /> (b) The volume of each type of used oil transported, <br /> identified by shipping description. <br /> (c) The facilities to which the used oil was transported, <br /> identified by name, address, telephone number, and EPA <br /> identification number. <br /> 6. Unregistered Hauler <br /> The owner or operator shall notify the Department in <br /> writing within 15 days when the facility receives any <br /> hazardous waste from- an unregistered hauler or if the <br /> facility receives a hazardous waste that was transported in <br /> a vehicle or container failing to display a valid <br /> certificate of compliance. m.n�y. .s. <br /> i5 i.{ <br /> H. Analysis of Waste <br /> 1. Upon the effective date 'ofl=this : �oer�mi^t', . tie owner or <br /> operator shall follow the writtenwas-te_y analysis plan as <br /> -r� <br /> described in the approved Operation Plan. <br /> 2. The analysis shall be repeated, as necessary, to ensure <br /> that it is accurate and up-to-date. As a minimum, the <br /> analysis must be repeated when the owner or operator is <br /> notified or has reason to believe that the process <br /> operation generating the hazardous wase has changed. <br /> 3. The owner or operator shall verify the waste analysis plan <br /> as part of the quality assurance procram. This quality <br /> assurance program will be in accordance with current U.S. <br /> EPA practices (Test Methods for Evaluating Solid Wastes; <br /> Physical/Chemical Methods SW-846 dated July 1982) or <br /> equivalent methods approved by the Department; and at a <br /> minimum ensure that the owner or operator maintain proper <br /> functional instructions, uses approved sampling, and <br /> -16- <br />
