SUPPLEMENTAL CONDITIONS <br /> FOR ENCROACHMENT PERMIT NO. 66856 <br /> The following are included as part of the conditions for approval of Encroachment Permit no 66856 <br /> 1 Comply with all requirements in the revised Section 100 of the Standard <br /> Specifications and Plans. <br /> 2 One lane shall remain open and accessible to traffic in each direction, provide a <br /> flagman when reduced to one lane <br /> 3 All work is restricted to nighttime hours of 10 pm and reopened by 5 am <br /> 4 All intersections and driveways shall remain open, and accessible to traffic in each <br /> direction at all times <br /> 5 All traffic control shall conform to the Caltrans Traffic Control, the use of a lighted <br /> arrow board is required <br /> 6 At the expense of the permittee, one compaction test, to be analyzed by a certified <br /> independent laboratory, is required every 100 feet of trench per 2 feet of lift, or as <br /> directed by the Public Works inspector <br /> 7 All trenching, backfill and paving shall conform to City of Stockton Standard Drawings <br /> 50, 50A and 50B The trench shall be overcut twelve (12) inches on either side, and <br /> repaved Pavement section shall be equivalent to the existing depth, but in no case <br /> less than 13 inches on Robinhood Drive <br /> 8 The use of control density backfill material (CDF) is required for all trenches eight(8) <br /> inches or less and shall be restricted to the use of structural controlled density fill per <br /> City of Stockton standard specification 19-3 066 The CDF mix design shall be <br /> submitted to the Public Works Department for review and approval prior to <br /> placement <br /> Failure to comply with the above conditions shall cause this permit to be immediately revoked <br /> Revocation notice may be by verbal notification by an authorized representative of the Public Works <br /> Director or by written correspondence <br /> EORG AMtNTRO S <br /> JUNIOR ENGINEER <br /> i <br /> 1 <br />
