The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Valley Rock Quarry as of April 07, 2020. <br /> Open violations from July 12, 2018 inspection <br /> Violation#5-Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for all chemicals are missing the correct max daily amounts. Please up <br /> date the max daily amounts.This information must be current and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6-Failed to complete and/or submit a site map with all required content. <br /> The site map is missing emergency shutoff locations for the propane tank. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not correct.The emergency response procedures has the wrong phone <br /> number listed on the local Unified Program Agency(UPA).
