The following is an itemized list of hazardous waste violations that have not been <br /> addressed for EMILS LIQUOR & SPORTS SHOP* as of May 22, 2018. <br /> Open violations from May 30, 2017 inspection <br /> Violation #104 -No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> Violation #110- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. <br /> Manifest number 002739465GBF (2015)was not found on site, but was found on the DTSC Hazardous Waste <br /> Tracking System website. Hazardous waste generators shall retain copies of all manifests signed off by the <br /> disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them <br /> readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain <br /> them on site, and submit copies to the EHD. <br /> Violation #111 - Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest number 002739465GBF (2015)was not found on site. Hazardous waste generators shall retain copies of <br /> all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy <br /> shall be retained and made readily available for review for three years. If the generator did not receive a copy of the <br /> manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a <br /> legible copy of the missing manifest, with some indication that the generator has not received confirmation of <br /> delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br /> Violation #302 - Failed to maintain required emergency equipment. <br /> Hazardous waste storage containers/drums were not found on site. All facilities shall be equipped with an internal <br /> communication or alarm system, a device capable of calling outside emergency help, spill control equipment, and <br /> decontamination equipment at all times. Immediately provide necessary spill equipment and maintain on site. <br /> Violation #608 - Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill of <br /> lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters <br /> for the last three years, maintain them on site, and submit copies to the EHD. <br /> Page 1 of 1 <br />
