The following #8 and 85 are , and#8,60 & 63, <br /> 85 are Minor - Notice to Comply. <br /> 8. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br /> generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br /> an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br /> evidence to the EHD by May 13,2011. <br /> 23. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br /> copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br /> EHD by May 13,2011. <br /> 60 & 63. Copies of hazardous waste disposal records for used oil were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for <br /> review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them <br /> on site, and submit copies to the EHD by May 13,2011. <br /> 85. Copies of Universal Waste disposal records were not found on site. A copy of the disposal receipts <br /> of Universal Waste, including but not limited to lamps, alkaline batteries, mercury containing switches, <br /> CRT, and electronic devices, shall be retained on site available for inspection. Immediately locate a <br /> copy of all Universal Waste disposal records for the last three years, maintain them on site, and submit <br /> copies to the EHD by May 13,2011. <br /> NOTES (The following are not violations) <br /> --The facility generated less than 5 tons of hazardous waste in 2010. Discussed with generator <br /> requirement to manage each type of Universal Wastes including aerosol cans, lamps, alkaline batteries, <br /> mercury containing switches, CRT, electronic devices, etc. in a closed container with a with a label <br /> identifying the content anc the start date of accumulation, and labelling of Hazardous Waste. Universal <br /> Wastes must be managec within 1 year of accumulation, and Hazardous Waste within 6 months. <br /> --Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by May 13,2011. <br /> ALL EHD STAFF TIME ASSOCIATEO WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY ISSUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD I Spector: Receive � Title: <br /> EHD 23-02-003 R 08/10/10 CONTINUATION FORM <br />
