The following is an itemized list of hazardous waste violations that have not been <br />addressed for AUTO PROS as of August17, 2018. <br />Open violations from March 10, 2016 inspection <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br />of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br />web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br />Immediately reactivate your State ID number CAL000367586 and submit evidence to the EHD. <br />Violation #113 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for 2013 were not found on site and the records for 2014 and 2015 are <br />incomplete. The hazardous waste label for used oil indicated pick ups on 1-9-15, 4-2-15, and 6-6-15. The hazardous <br />waste label for used coolant indicated pick ups on 4-18-14 and 4-7-15. The hazardous waste disposal receipts for <br />these pick ups were not found on site during the inspection. Hazardous waste generators shall retain copies of all <br />manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for <br />three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for <br />the last three years, maintain them on site, and submit copies of the missing disposal records to the EHD. <br />Violation #115 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years. <br />Bills of lading or manifests for the management of lead acid batteries for the last three years found on site are <br />incomplete. The 2013 bills of lading or manifests for the management of lead acid batteries could not be found <br />during the inspection. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a <br />copy of all bills of lading or manifests for lead acid batteries for the last three years, maintain them on site, and <br />submit copies of the 2013 bills of lading or manifests for lead acid batteries to the EHD. <br />Violation #117 - Failed to report program data electronically. <br />A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br />made. Beginning January 1, 2013, all businesses are required to submit all new (or any changes to existing) <br />hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br />waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br />fields. <br />Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br />Dark oil stains were found on the shop floor near the middle bay roll up door. Dark oil stains and contaminated <br />absorbent were found on the shop floor next to the used oil tank and used coolant drum. Facilities shall be <br />maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or <br />surface water which could threaten human health or the environment. Immediately clean up the dark oil stains and <br />contaminated absorbent, and manage according to Title 22 hazardous waste regulations. Submit a statement and <br />supporting documentation explaining how this waste was managed. <br />Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br />hazardous waste. <br />The drum of hazardous waste coolant was left open. All hazardous waste containers shall be closed at all times <br />except when adding or removing waste. Immediately close these containers and ensure all hazardous waste <br />containers are closed when not adding or removing waste. <br />Violation #601 - Stored hazardous waste onsite greater than 180 days. <br />The hazardous waste label for the used oil tank listed an accumulation start date of 4-2-15 and a pick up date of <br />3-1-16. This indicates that the used oil was stored on site longer than 180 days. The hazardous waste label for the <br />used coolant drum listed an accumulation start date of 4-7-15 and a pick up date of 2-26-16. It also listed an <br />accumulation start date of 8-11-14 and a pick up date of 4-7-15. This indicates that the used coolant was stored on <br />site longer than 180 days on two separate occasions. Facilities who generate less than 1000 kg of hazardous waste <br />per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. <br />Ensure that any hazardous waste stored on site does not exceed the allowable time limits. <br />Page 1 of 2 <br />
