Lodi, CA <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />including the hazardous class and United Nations/North America (UN/UA) identification number, if <br />applicable. (E) The name, address, and identification number of the authorized facility to which the <br />hazardous waste will be transported. (F) The transporter's name, address, and identification number. <br />(G) The driver's signature. (H) A statement, signed by the generator, certifying that the generator has <br />established a program to reduce the volume or quantity and toxicity of the hazardous waste to the <br />degree, as determined by the generator, to be economically practicable. Correct immediately by <br />ensuring that your own active EPA ID# CAL000337354 is used to complete the consolidated manifests. <br />132. Anthony Morphis stated that the hazardous waste tank and containers are inspected daily but that <br />the daily inspections of the hazardous waste used oil tank are only documented weekly. (a) The owner <br />or operator shall inspect, where present, at least once each operating day: (1) overfill/spill control <br />equipment (e.g., waste -feed cutoff systems, bypass systems, and drainage systems) to ensure that it is <br />in good working order; (2) the aboveground portions of the tank system, if any, to detect corrosion or <br />releases of waste; (3) data gathered from monitoring equipment and leak -detection equipment, (e.g., <br />pressure and temperature gauges, monitoring wells) to ensure that the tank system is being operated <br />according to its design; (4) the construction materials and the area immediately surrounding containment <br />structures (e.g., dikes) to detect erosion or signs of releases of hazardous waste (e.g., wet spots, dead <br />vegetation); and (5) for uncovered tanks, the level of waste in the tank, to ensure compliance with <br />section 66265.194(b)(3) ... (c) The owner or operator shall document in the operating record of the <br />facility an inspection of those items in subsection (a) and (b) of this section. Correct immediately by daily <br />conducting and documenting the hazardous waste tank inspections. <br />605. The hazardous waste used oil tank most recent accumulation start date is listed as October 24, <br />2011. Used oil consolidated manifests indicate the most recent used oil pick up occurred August 8, <br />2012. All hazardous waste tanks shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. accumulation start date <br />Immediately update the accumulation start date and ensure that all hazardous waste tanks are marked <br />with all the required information. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by October 6, 2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY MEA EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Recei y: <br />Titl . <br />Aris Cacapit (209) 468-3453 <br />EHD 23-02-003 Rev 04!19/12 ( CONTINUATION FORM <br />
