MERRILL WEST HIGH SCHOOL o Uniform Hazardous Waste Manifest(s): We cannot locate any past manifests because the address for the removal was 1975 W. Lowell. We had been previously told that because the high school was connected to the DSC site (despite different addresses), the removal could be made under the existing EPA number for the DSC. We have now learned this is not correct and will use the EPA number for the West High location. In the future, all hazardous waste manifests will be kept on-site in the main office in a clearly identified binder. o Universal Waste Disposal Receipts: Universal Waste was transported to the District Education Center and disposed of accordingly. A copy of the disposal record is attached. o Hazardous Waste Training Records: Sign -in sheets are attached. o Statement of How Each Violation Was or Will Be Corrected, including management of wastes: ✓ #7: The waste was removed from the site on March 1, 2011. Manifest is attached. ✓ #8: WHS was issued EPA number CAL000273993. ✓ #15: Communications regarding alarm systems, spill control, and decon equipment is now being kept in the main office. The eyewash/shower is installed correctly at the West High swimming pool. Both the eyewash and shower drain indirectly to a sanitary sewer drain inlet in the concrete just in front of the eyewash/shower assembly. ✓ #26 & 27: Inappropriate containers have been permanently removed and will not be used to store hazardous waste in the future. Containers are now clearly marked and identified and will remain closed. Waste was removed by Evergreen Environmental on December 1, 2010.
