SANJ O A Q U I N Environmental Health Department <br /> J `Il I 'v <br /> Complaint Record Daily Time and Activity Summary from 6/21/2013 to <br /> 6/21/2020 for CODLY3A52 located at <br /> CORRAL HOLLOW RD-580 EB ON-RAMP <br /> Activity Date Recorded by <br /> 02/05/2020 Received spill information from Elianna Florido, night-time spill person. A PE 2546 ALANIZ <br /> 180 min mechanical failure had occurred with a tractor trailer that caused an unknown SC 544 <br /> amount of coolant to be released. <br /> 8:42 a.m.: Called Marsha Thompson, ERTS, 440-349-2700, and she informed me <br /> that she was not sure where the spill was (north or southbound). She also did not <br /> have the contact information for anyone on site. Marsha informed me that the <br /> driver had called her and said that approximately %2 a gallon of coolant was <br /> released onto the asphalt. <br /> 8:54 a.m.: Called Mayra Salmeron, Fed Ex Ground, 209-830-2580. She said that <br /> the spill happened on the southbound on ramp to 580 near the entrance. I asked <br /> her if anyone had called Caltrans and she said no. She also said that going <br /> forward I would need to speak with the environmental department at <br /> 412-262-7375. Mayra's email is <Mayra.salmeron@fedex.com>. <br /> 9:00 a.m.: Called Juan Armendarez, Caltrans Hazmat, 209-948-3723. He did not <br /> have any concern with the small amount that spilled. He informed me to call him if <br /> did find out it was a large spill. <br /> 9:06 a.m.: Called Mike Hardy, Clean Harbors cleanup, 916-662-1158. Left a <br /> voice message letting him know that I was going to be responding to the site. <br /> 9:12 a.m.: Called Mike's supervisor, Scott Murtishaw, 707-410-8242. Informed <br /> him what I had found out and that I needed to be there for the final clean up. <br /> 9:27 a.m.: Called the environmental department for Fed Ex, 412-262-7375 and <br /> left a voice message. <br /> Created complaint. Fill all informatio <br /> 10:06 a.m.: Called Scot Hardy who let me know he was going to be at the site <br /> around 11:00 a.m. Informed him of what I needed to see for final clean up. <br /> 10:12 a.m.: Spoke with Izzy Cross-Najafi, FedEx environmental department, <br /> 412-859-5546, <Izzy.cross-najafi.osv@fedex.com>. She said that her and Brett <br /> Johnson with ERTS would be the people to contact going forward. Explained to <br /> her that I would be emailing her because she is the responsible party (explained <br /> procedures). Brett's contact information is 441-349-2700 ext 313 and <br /> <bjohnson@ertsonline.com>. <br /> 10:56 a.m.: Received a call from Mike letting me know that he was almost at the <br /> site. He had a couple of questions regarding the location. Let him know what I <br /> knew. <br /> 11:22 a.m.: Called Mike for an update. He said that he found the spill location (on <br /> ramp east bound). However, he could not clean it up because he did not have any <br /> traffic cones. The spill was on the shoulder of the road. Mike is going to get <br /> cones. He said that he would be done in approximately 90 minutes. He <br /> messaged me photos of the spill location. <br /> Called Mike who said that the cleanup was going to start. <br /> Left for the site at approximately 1:00 p.m. and arrived at 1:38 p.m. <br /> The cleanup was completed by the time I arrived on site. Met with Mike. The <br /> coolant was cleaned up using absorbent. Approximately 1/4 of a 30 gallon drum <br /> of used absorbent was generated. The drum was labeled as non-hazardous <br /> waste and transported on a uniform hazardous waste manifest to the Clean <br /> Harbors yard. No storm drains or waterways were observed affected by the <br /> release. A profile was already on file for the coolant. <br /> Call ended at 2:24 p.m. <br /> Page 2 of 3 <br />
