SAN J O A Q U I N Environmental Health Department <br /> WASTE DISCHARGE/SPILL RESPONSE <br /> NOTICE TO ABATE <br /> Responsible Party: FEDEX GROUND Date: Feb 05, 2020 <br /> RP Mailing Address: 1000 FEDEX DRIVE Record ID#: CODLY3A52 <br /> MOON TOWNSHIP, PA 15108- Program Element: 2546 <br /> Spill Location: 0 EB 580-CORRAL HOLLOW RD, Release/Spill Response(excluding Joint Team) <br /> NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br /> A tractor trailer had a mechanical failure which caused the release of half of a gallon of coolant onto asphalt. <br /> OBSERVATIONS: <br /> The cleanup was completed by the time I arrived on site. Met with Michael Hardy, Field Services Foreman for Clean <br /> Harbors. The coolant was cleaned up using absorbent. Approximately 1/4 of a 30 gallon drum of used absorbent was <br /> generated. The drum was was labeled as non-hazardous waste and transported on a uniform hazardous waste manifest to <br /> the Clean Harbors yard. No storm drains or waterways were observed affected by the release. <br /> VIOLATIONS: <br /> • Discharge of any waste, including sewage <br /> (CA Health and Safety Code 5411) <br /> Discharge of waste or sewage to water or within 150 ft. of waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> Improper disposal of a hazardous waste <br /> (CA Health and Safety Code 25189.5) <br /> Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> ° Other Violation <br /> CORRECTIVE ACTIONS/ORDER: <br /> ° Investigate and abate contamination under the oversite of the appropriate lead agency. <br /> • Submit copies of hazardous waste manifests/disposal records to the EHD. <br /> Submit final disposal records and waste determination documents to the EHD. <br /> ° Other Correction <br /> COST RECOVERY: All EHD time associated with this Spill Response is billed to the responsible party at the current hourly <br /> rate.
