Hammer Lane, Stockton, CA 95219 <br /> The San Joaquin County Environmental Health Department(EHD)has completed the <br /> review of the application for permit to replace the following at the above noted address: <br /> • Regular unleaded(87) leak detector <br /> • Mid-grade unleaded(89) leak detector <br /> • Premium unleaded (91) fill spill bucket <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Toua Yang at(209) 468-3459 to arrange the following inspections <br /> appointments (a 48-hour notice is required for inspection appointments): <br /> a. Leak Detector Test: No additional testing is required at this time. EHD <br /> was onsite at the time of installation. The new leak detectors were tested <br /> and deemed functional. <br /> b. Spill Bucket: Verify minimum 5-gallon capacity and pass a 60-minute <br /> Lake Test or equivalent. Proper drainage or removal must also be <br /> verified. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. All components installed must be certified by an independent testing organization <br /> and bear the appropriate mark as required by the independent testing organization. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br />
