The newly installed fill sump does not bear the required approval label and a <br /> special inspector must be hired to certify the sump is listed. The annular space of the tank <br /> failed secondary containment testing and a valid repair and retest has not been performed. <br /> Liquid remains in the UST and the monitoring panel is in alarm, hence the UST is not <br /> being monitored. <br /> Recent phone conversations and voicemail messages from the UST Owner and Operator <br /> indicate that a decision has not been made by both parties to remove the UST or to <br /> perform a repair/retrofit of the UST. <br /> Further inspections by this department will cease until EHD receives a written notice <br /> signed by both parties on the intent either to remove or repair/retrofit the UST. <br /> Respectfully, <br /> *�t L& <br /> Michelle Le, REHS <br /> cc: <br /> David Irey-Deputy District Attorney <br /> Kevin Eckman-UST Operator <br /> John Leonard-UST Owner <br /> Marion McDonald-Property Owner <br />
