Contact the local fire department, building department and San Joaquin Valley Air Pollution <br />Control District for applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to EHD prior to <br />commencing work. <br />3. Contact Steven C. Shih at (209) 468-9850 to arrange inspection appointments; 48-hour <br />notice for inspection appointments is required. Contact EHD for the following inspections: <br />(a) Secondary Containment test. <br />(b) Monitoring certification include under dispenser containment sensors. <br />(c) Complete and submit form C. A copy is enclosed. <br />4. Permit expires within 90 days.
