Copies of hazardous waste disposal records for 2008 were not found on site. -- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. * Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies of the missing 2008 receipts to the EHD by June 26, 2011. 82,83,84. Unable to determine the length of storage of 13 waste CRTs in the shop storage room, and 32 CRTs outside the shop in a fenced-in area. The CRTs were not marked to identify the type of universal waste and the start date of accumulation. The CRTs outside the shop and two CRTs in the storage room were not contained in a manner that prevents breakage and release to the environment. -- Universal Wastes shall be stored in a closed container that prevents breakage and release to the environment. The container shall be labeled to identify the type of Universal Waste and marked the initial date of accumulation. Universal Waste shall be managed within one year of storage and retained on site a copy of the disposal receipt for at least 3 years. * Immediately dispose of the above Universal Waste - lamps and submit a copy of the disposal receipt to EHD by June 26, 2011. A pamphlet for the Small Hazardous Waste Disposal Facility has been supplied to the generator during this inspection 85. Copies of Universal Waste disposal records were not found on site. --A copy of the disposal receipts of Universal Waste, including but not limited to lamps, alkaline batteries, mercury containing switches, CRT, and electronic devices, shall be retained on site available for inspection. * Immediately locate a copy of all Universal Waste disposal records for the last three years, maintain them on site, and submit copies to the EHD by June 26, 2011. NOTES Facility generated 2331 pounds (333 gallons) of hazardous waste in 2010. Facility is a technical school with a shop that generates auto maintenance type of hazardous waste. Submit the following to EHD by: June 26, 2011 1) A completed copy of the (provided) Return to Compliance (please keep original for your records). 2) A written statement how each violation was corrected 3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
