Copies of hazardous waste disposal records for 2008 were not found on site. <br /> -- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br /> all receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. <br /> * Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br /> and submit copies of the missing 2008 receipts to the EHD by June 26, 2011. <br /> 82,83,84. Unable to determine the length of storage of 13 waste CRTs in the shop storage room, and <br /> 32 CRTs outside the shop in a fenced-in area. The CRTs were not marked to identify the type of <br /> universal waste and the start date of accumulation. The CRTs outside the shop and two CRTs in the <br /> storage room were not contained in a manner that prevents breakage and release to the environment. <br /> -- Universal Wastes shall be stored in a closed container that prevents breakage and release to the <br /> environment. The container shall be labeled to identify the type of Universal Waste and marked the <br /> initial date of accumulation. Universal Waste shall be managed within one year of storage and retained <br /> on site a copy of the disposal receipt for at least 3 years. <br /> * Immediately dispose of the above Universal Waste - lamps and submit a copy of the disposal receipt <br /> to EHD by June 26, 2011. A pamphlet for the Small Hazardous Waste Disposal Facility has been <br /> supplied to the generator during this inspection <br /> 85. Copies of Universal Waste disposal records were not found on site. <br /> --A copy of the disposal receipts of Universal Waste, including but not limited to lamps, alkaline <br /> batteries, mercury containing switches, CRT, and electronic devices, shall be retained on site available <br /> for inspection. <br /> * Immediately locate a copy of all Universal Waste disposal records for the last three years, maintain <br /> them on site, and submit copies to the EHD by June 26, 2011. <br /> NOTES <br /> Facility generated 2331 pounds (333 gallons) of hazardous waste in 2010. Facility is a technical school <br /> with a shop that generates auto maintenance type of hazardous waste. <br /> Submit the following to EHD by: June 26, 2011 <br /> 1) A completed copy of the (provided) Return to Compliance (please keep original for your records). <br /> 2) A written statement how each violation was corrected <br /> 3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME EHD'S CURRENT HOURLY RATE. <br /> EHD I ect Recei By: Title: <br /> EHD 23-02-00 Rev 08/10/10 CONTINUATION FORM <br />
