A one gallon container of used oil was observed in a trash can. The operator removed the container <br /> from the trash. The container was not labeled. <br /> All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label this container and ensure that the container is marked with all the required <br /> information. Since it cannot be determined how long the used oil has been on site, immediately contact <br /> a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the <br /> manifest to the EHD by July 28, 2011. <br /> Notes <br /> 1. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 28, 2011. <br /> 2. The facility has universal waste on site: two paper shredders, one printer, one laptop computer, one <br /> fax machine, one LCD monitor and 22 fluorescent lamps. This waste cannot go into the solid waste <br /> dumpsters. Send this waste to an universal waste handler or universal waste destination facility. <br /> Maintain a log showing how the waste was handled or keep the recycling /disposal receipts. <br /> 3. The operator was provided with the San Joaquin County Small Business Hazardous Waste disposal <br /> facility brochure. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: ( Title: <br /> �iI I l�C <br /> EHD 23- -0 3 Rev 08/ 0110 ,_ ��p�y`� CONTINUATION FORM <br />
