This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately <br /> contact DTSC and reactivate your EPA ID number and submit evidence to the Environmental Health <br /> Department (EHD) by December 30, 2010. A form has been provided. <br /> Class 2 repeat violations <br /> 20. Three 55-gallon drums of waste oil were observed at the facility. One drum was labeled "hazardous <br /> waste" only. Two drums were not labeled. All hazardous waste containers shall be marked with the <br /> following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long the three 55-gallon drums have been on site, <br /> immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and <br /> submit a copy of the manifest to the EHD by December 30, 2010. <br /> 23. One 55-gallon drum of used oil filters was at the facility and labeled "drained used oil filters" only. <br /> Used oil filters must be stored in a closed, rainproof container, labeled as "drained used oil filters," and <br /> marked with an accumulation start date. Immediately add the accumulation start date to the label and <br /> ensure that all used oil filters are managed in this way. <br /> 23. One 55-gallon drum of used oil filters was at the facility and labeled drained used oil filters only. . <br /> Facilities that generate up to one ton of used oil filters per year may store the filters up to one year. <br /> Immediately recycle these filters under a bill of lading and submit a copy of the bill of lading to the EHD <br /> by December 30, 2010. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Insp Received By: Title: 7: <br /> E 3-02-003 Rev 08/10/10 CONTINUATION FORM <br />
