This <br /> information was required to be submitted by April 18,2011. This information has not been received resulting in a <br /> non-compliant status for this facility. <br /> Failure to correct the violations will result in continuing/additional violations. <br /> Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is billed at the <br /> current hourly rate($122).
