The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 3. Once failures are discovered for piping, work must stop, and an updated scope of <br /> work must be submitted and approved before work resumes. <br /> 4. Contact Michelle Henry(209) 953-7699 to arrange the following inspection <br /> appointment (a 48-hour notice is required for inspection appointments): <br /> a. Lake test on UDC 5/6, 7/8 Diesel Sump, and Premium(91) sump <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board(may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested, repaired, or serviced. <br /> And <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br />
