The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator monthly inspection reports since 12/2008 to present. Designated operator, Robert Barnhart, noted the date as 12/10/2008. The actual secondary containment testing last occurred 11/18/08. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Repeat Violation. 206. The designated operator, Robert Barnhart, failed to verify that all employees training is current. Upon arrival to the facility, only the clerk Nasrat Amani was present. His last training was conducted 01/2010. Another employee, Ken Cook, last employee training was last conducted 03/2010. The manager Avinash Sharma last done 07/2010. A new female employee, Ruchi, who has been since approximately beginning of the year is due for an initial training within 30 days of her employment. During the monthly inspection, the designated operator shall verify that all facility employees have been trained. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Correct by having the Designated Operator submit a copy of the current employee training records to the facility and another copy to SJC EHD. Repeat Violation. Submit to SJC EHD within 30 days (07/02/2012)- 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement 3. any supporting or requested documents: secondary containment test notification, Designated Operator employee training, financial responsibility documents ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
