The designated operator failed to check and note the correct test dates for the secondary containment testing on the designated operator monthly inspection reports for approximately two and half years from 12/10/08 to present. The designated operated documented the date as 12/10/08. The actual secondary containment testing last occurred 11/18/08. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 23c. The designated operator failed to verify that all employees training is current. Upon arrival to the facility, only the clerk Nasrat Amani was present. His last training was conducted 01/22/10. Another employee, Ken Cook, last employee training was last conducted 03/30/10. The manager Avinash Sharma next training is due by 07/2011. A new female employee who has been working for approximately one week is due for an initial training by the end of the 5/2011. During the monthly inspection, the designated operator shall verify that all facility employees have been trained. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit a copy of the Designated Operator employee training log for the annual refresher for the previous employee and the newly hired. Submit to SJC EHD within 30 days (06/16/11): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for the above violations 3. requested/ supporting documents Notes- - Provided Facility Form A, three Tank Form B, financial responsibility documents (Certification of Financial Responsibility and Letter from Chief Financial Officer), Monitoring Plan, Response Plan ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
