Secondary containment testing was performed on June 20, 2012, and a test report was submitted <br /> to the EHD on August 9, 2012. A copy of the test report must be submitted within 30 days of the test. <br /> Take all necessary precautions to ensure that a copy of the test report is submitted to the EHD within 30 <br /> days of the test. <br /> 204. The designated operator failed to check and note the correct test dates for the secondary <br /> containment test on the August 2012 and September 2012 designated operator monthly inspection <br /> reports. The test date was listed as December 10, 2008, but should have been listed as June 20, 2012. <br /> During the monthly inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. Ensure <br /> that designated operator performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> 205. The 2013 March, April, and May designated operator monthly inspection reports were not found on <br /> site. Designated operator monthly inspection reports for the previous twelve months shall be retained on <br /> site. Locate and ensure that copies of the previous twelve months of designated operator monthly <br /> inspection reports are maintained on site. Submit copies to the EHD by July 4, 2013. <br /> 303. The 87 turbine sump sensor was located approximately sixteen inches away from the lowest point <br /> of the sump, part way up the sump saddle, and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> During the inspection, the technician repositioned the sensor to the lowest point of the sump to detect a <br /> leak at the earliest opportunity. Take the necessary precautions to ensure that all sensors are located at <br /> the lowest point to detect a leak at the earliest opportunity. <br /> 306. A fuel leak was found at the diesel filter in dispenser 1/2. All primary containment must be product <br /> tight. During the inspection the technician removed the diesel from the dispenser pan and tightened the <br /> diesel filter.
