PHS-EHD would like to remind you that, in order <br /> to be acceptable, the line test result must include, but not <br /> limited to, the following <br /> 1 . Name and address of the facility. <br /> 2 . Name and address of owner or operator. <br /> 3 . Test information. <br /> 4 . Tank and lines information. <br /> 5 . Test data, including leak rate . <br /> 6 . Tightness certification. <br /> 7 . Tester' s signature and test date . <br /> PHS-EHD will not be able to accept any test result without the <br /> above minimum information.
