r
26 July 2011
AGE-NC Project No. 95-0137
All original well construction materials will be removed (including well casing, screen, filter
pack, and annular seal) to ensure that no residual contamination is sealed within the
abandonment borehole. The remaining borehole will be backfilled to surface grade with
neat cement and covered concrete mix or native dirt, 'depending on the surrounding
surface material. #
x -
Well boxes and caps will be removed to expose the well casings of non-impacted wells.
The well heads will be fitted with a pressure tight adapter,'connected to a hose extending
from a grout mixer. A 'thick grout mixture will be pumped into the well casing under
approximately(10 pounds per square inch (psi) of pressure. The well will then be
over-drilled to approximately 3 feet bsg using eight ;inch .`or or ten-inch diameter
hollow-stem augers and a hole one-foot larger will be excavated. The top 2.5 feet of the
well casing will then be cut and grout-will be used to'[fill thi,e base of the boring to
approximately 0.5 feet above the top of the well casing. The remaining borehole will be
backfilled to surface grade with dirk or cement and asphalt; depending on the surrounding
surface material.
Soil cuttings generated during drilling activities and rinseate will be containerized in landfill I
appropriate disposal container, 55-gallons drums and stored on-site for disposal, in an area
lacking public access.
Upon completion of field work and disposal of soil-cuttings, a,Mell Destruction Report
describing well destruction activities and procedures, including documentation of waste
disposal from well destruction and other activities, will be"prepared and submitted to the
EHD.
