(Ground Zero), on September 19,2003. San Joaquin County
Environmental Health Department (F-HD) has received well permit application for the
installation of the proposed soil vapor extraction well, monitoring well 1\4W-10, MW-11,
MW-12, and MW-13. The permit application did not include the additional monitoring well
with screen section targeted for the sand layer at approximately 120 feet to 130 feet below
surface grade (similar to the construction of Monitoring Well MW-9B) in the immediate
vicinity of the former domestic well which was previously discussed and agreed to in a
telephone conversation with Ground Zero on July 31, 2003. EHD requested this additional
well be included in the permit application and be installed in the same mobility with.the
other above-mentioned proposed wells.
In a telephone conversation with Ground Zero on September 23, 2003, EHD.agreed with I
Ground Zero's proposal to construct the additional monitoring well using 4-inch diameter
PVC casing for the reason that the well could be doubled as a possible point of ground
water extraction, in the future. i
Please direct any questions to Jeffrey Wong at (209) 468-0335. i
Donna Heran, REHS, Director
Environmental Health Department
Jeffrey Wong, Senior REHS
LOP/Site Mitigation Unit IV
Cc: CRWQCB, Central Valley Region —James L.L.
