The replacement well was installed on October 18, 2001. Environmental <br /> Health Department (EHD) has repeatedly requested the submission of the analytical results <br /> of the new well over multiple telephone conversations in November and December 2001 <br /> with your consultant Groundzero Analysis, Inc. (Groundzero) and Clark Well, Inc. (Clark). <br /> The results were received by this office via fax on December 17,2001. Microbiology <br /> analyses of the groundwater collected from the replacement well indicated the presence of <br /> coliform. <br /> In a telephone conversation with EHD on December 17, 2001, Clark expressed a second <br /> microbiological test will be performed after the replacement well has been chlorinated_ <br /> EHD has not received the results of the second microbiological test to date. <br /> In a telephone conversation with EHD on January 7, 2002, Clark expressed that the second <br /> microbiology test also failed to pass. Clark stated the replacement well will be chlorinated <br /> again and another microbiology test performed by January 11, 2002. <br /> Submit the results of the new microbiology test to EHD no later than January 25, 2002. <br /> Failure to submit the results may jeopardize your reimbursement status with the <br /> Clean Up Fund. <br /> As a responsible party,you were required to destroy the existing contaminated water supply <br /> well by Central Valley Regional Water Quality Control Board. In a letter dated May 26, <br /> 2000,Groundzero proposed the destruction schedule of the contaminated well by August <br /> 31, 2000. In a letter dated August 22, 2001 from EHD,you were given the revised schedule <br /> to destroy the well of November 25, 2001. You were also directed to analyze and submit the <br /> results of groundwater samples from the contaminated well for bacteriology, nitrates, and <br /> nitrites by September 10,2001 and on a monthly basis thereafter in accordance to California <br /> Code of Regulations,Title 22,Section 64421, 64423(x), and 64432(a).
