Upon completion of treatment and cessation of the <br /> discharge, the Discharger may request official termination of coverage under this General Order <br /> from the Executive Officer. Failure to do so will result in the Discharger's continuing responsibility <br /> for payment of the annual fee associated with this Order. <br />
