A Report of Waste Discharge and filing fee will be submitted by each proposed Discharger, providing information on the discharge as detailed in Attachment A. If the Executive Officer folds that the proposed discharge qualifies for coverage under this Order, the Discharger shall be issued a Notification of Applicability statement. Upon completion of treatment and cessation of the discharge, the Discharger may request official termination of coverage under this General Order from the Executive Officer. Failure to do so will result in the Discharger's continuing responsibility for payment of the annual fee associated with this Order.
